We ask that prior to your visit you confirm individual physician’s network participation. Please check with your insurance company by calling the number on the back of your insurance card to verify physician participation in your specific “plan”.  Not every physician participates in every plan that a company offers so, when calling the insurance company specifically mention your “plan name” to insure this coverage.
